Understanding Care Options Across West-Central New Jersey
This guide helps seniors, families, and healthcare professionals—social workers, case managers, and medical providers—navigate the full range of senior living and post-acute care options across the Princeton–Mercer County corridor. It is a clear, practical resource for matching individual needs with the right level of support.
Most residents know our major hospitals—Penn Medicine Princeton, RWJ Hamilton, and Capital Health in Hopewell and Trenton. Yet as we or our loved ones age, new questions arise: What comes after the hospital? Recovery often requires a different kind of setting—specialized, residential, and focused on continuity of care.
Beyond the hospital system lies an interconnected network of senior living and medical communities essential to long-term wellness. Understanding how these settings differ— and how they work together—helps families and professionals make confident, informed choices.
